  • I will report a lost or stolen card, or any change of personal information (name, address, phone, email) immediately.
  • I will comply with all library rules and policies including, but not limited to, Borrowing Rules and Internet Use Policy
  • I understand that there will be charges for lost, damaged, and stolen library materials and may be overdue charges.
  • I understand that the library provides access to a broad range of resources and that it is my responsibility to judge for myself and for my children or minor dependents, what resources are appropriate for my/our personal use.

Upload a valid photo ID (i.e. Driver's license, state ID card) if you are 18 or older. Applicants under the age of 18 will need the signing adult to upload their valid photo ID. Upload Requirements: One file only, 2 MB limit, and Allowed types: gif, jpg, jpeg, png.

If your valid photo ID does not have your current address please upload proof of your current address (i.e. Driver's license, state ID card, recent mail, checkbook, utility bill). Proof of current address needs to have your name and your address on it. Upload Requirements: One file only, 2 MB limit, and Allowed types: gif, jpg, jpeg, png.
